How to File A Claim

Claims Procedure: Please submit your claim within 30 days from the date after discharge from hospital/ of consultation.

1. Bring the Happy Tails Claim Form during your visit to the Vet clinic. You can download the form here.
2. Request the treating Vet to complete the “Medical” section of the form. The fee for obtaining the medical report shall be borne by you.
3. Mail the completed and signed claim form to MSIG within 30 days of the happening of such an event.
4. Please include the following documents in your claim submission: MSIG aims to settle your claim within 14 business days on receipt of full and complete documents including investigative reports, if any, to assess the claim.
   Further information may be needed.
   1. Records of all required vaccination
   2. Pet’s medical records, including the vets' notes
   3. Original final bills or receipts　
5. If you are filing a claim for the first time, please include the following documents:
   1. Pet’s clinical medical examination report for insurance enrollment
   2. Records of all required vaccinations
   3. Latest medical records, if any
6. Mail your completed form and documents to:
